The University of Coimbra, together with the Coimbra Local Health Unit, is developing and testing an innovative hybrid consultation model for people with chronic vascular disease, starting with patients suffering from peripheral arterial obstructive disease and their informal caregivers. The model combines in-person and remote care, including telemonitoring, digital education, and structured follow-up led by nurses with multidisciplinary support. The initiative aims to improve treatment adherence, ensure continuity of care, prevent complications, and promote healthier lifestyles while strengthening patient and caregiver self-management.
